We regret to inform you that despite our best efforts and appeals, Sutter Gould Medical Foundation has decided not to renew our contract with your Primary Care Provider. As a result, we will no longer be able to provide primary care services if Sutter Gould Medical Foundation is your primary care network, effective immediately.
We would be happy to provide you or your new provider with medical records as needed. To obtain your medical records, please contact your new primary care provider if you have one or submit a records request on our website via the chatbot on the bottom right corner.
We are disappointed by Sutter Gould’s decision and encourage you to contact your health plan to request continued care with your former primary care provider. Alternatively, you may choose to switch to a health plan that is in-network with our primary care providers.
We contract with most major insurance plans and IPAs and would welcome the opportunity to continue providing you with high-quality care through one of our in-network plans. Please email us at info@healthyuclinics.com if you would like more information on the plans that we are currently contracted with in your area.
