We are writing to inform you that the HealthyU Clinics location at 12100 N Dysart Rd, Suite 105, Surprise, AZ 85379 will be relocating to our HealthyU Clinics Peoria location, with services at the Surprise office ending effective April 19, 2026. This transition allows us to continue providing you with the same high-quality care in a consolidated location.
Our dedicated care team, including Krista Pellicore, MD and Justin Singh, DPM, will continue to provide care for you at our Peoria clinic, ensuring continuity and support throughout this transition.
Your health remains our top priority. You may continue care with your current provider at their new location or choose from other nearby HealthyU Clinics locations:

If you choose to continue your care outside of HealthyU Clinics, you may request that your medical records be transferred. Original records will be retained by HealthyU Clinics.
