HealthyU Clinics remains fully committed to providing a wide range of primary care services, and any care and treatment that you have received will continue to be available to you. Should you wish to continue your current care plan with another one of our dedicated HealthyU providers, you may schedule an appointment with a provider nearby, such as Robert Claflin, DO or Christine Ercoline, PA-C at our Gilbert-Baseline clinic (2353 E Baseline Rd, Gilbert, AZ 85234), or with Ricardo LaGrotta, MD or Christine Ercoline, PA-C at our Chandler clinic (3200 N Dobson Rd, Suite B1, Chandler, AZ 85224).

Patients who wish to be treated elsewhere may have a copy of their medical records transferred. Original records will be retained by HealthyU Clinics.
